🔹 The Case: A High-Risk Multinodular Goitre with Retrosternal Extension

🔸 A 64-year-old woman was referred to Dr. Aneesh Sugunan (Surgical Endocrinology) with a recurrent multinodular goitre.
🔸 The goitre had an extensive retrosternal extension into the anterior mediastinum, reaching as far as the tracheal carinal bifurcation and lying between all major thoracic vessels.
🔸 Given the complex anatomical challenges and the risk to major thoracic structures, the case required meticulous planning and a multi-specialty approach.


🔹 The Surgical Approach: A Precise & Coordinated Effort

Total Thyroidectomy via Cervical Approach – The primary thyroidectomy was performed with a careful cervical approach to ensure complete gland removal while safeguarding vital structures.
Mediastinal Extension Removed via Hemi-Sternotomy – Given the deep thoracic involvement, a hemi-sternotomy was required to safely excise the mediastinal portion of the goitre.

This highly intricate procedure was made possible through the seamless collaboration of multiple specialized departments.
