A few weeks ago, our very own Dr. Jino Sebastian and his dedicated team performed a complex McKeown esophagectomy, a procedure we briefly reported last Thursday. Today, we are thrilled to share a remarkable update: the patient came for a follow-up review and is doing extremely well. This outcome is a testament to the exceptional skill and unwavering commitment of our surgical and medical teams.

The Surgical Feat

The McKeown esophagectomy, a highly intricate procedure, involved several critical steps:

  1. Thoracoscopic Esophagectomy: This minimally invasive procedure involved removing the esophagus through small incisions in the chest.
  2. Laparotomy: An abdominal incision was made to create a gastric conduit, which is necessary to replace the esophagus.
  3. Neck Incision and Anastomosis: Finally, an incision in the neck allowed for the connection (anastomosis) of the gastric conduit with the hypopharynx.

This multi-stage surgery required precision, coordination, and advanced surgical skills, all of which were exemplified by Dr. Jino and his team.

Specialized Anesthesia

The anesthesia required for this procedure was highly specialized, particularly involving one-lung ventilation to provide a clear operating field for the thoracic surgery. Our Team Anaesthesiology, led by Ashu and Jithin, meticulously followed enhanced recovery protocols, ensuring the patient remained stable and comfortable throughout the procedure.

Overcoming Postoperative Challenges

Postoperatively, the patient faced hypoxia on POD (Post-Operative Day) 2, posing a significant challenge. Dr. Jino, displaying relentless dedication, worked tirelessly to manage the patient's condition. The patient’s inability to cough out secretions necessitated an innovative approach to clear the airways.

The Lifesaving Bronchoscopy

In a critical moment, our Pulmonology team responded promptly to an urgent call from Dr. Jino. They performed a bronchoscopy and discovered mucus plugs obstructing the bronchi, which were then successfully washed out. This intervention led to a dramatic improvement in the patient’s condition, highlighting the importance of interdisciplinary collaboration in healthcare.
